Bakerloo were formed as The Bakerloo Blues Line in February 1968, in Tamworth, Staffordshire and split in Summer 1969. The Line-up I have is:
Keith Baker drums (born 14.4.1948, in Birmingham, Warwickshire) (ex May Blitz, to Warm Dust, Summer? 1969? - May? 1970? Uriah Heep, May - Oct 1970, then presumably quit music). After Keith left The Bakerloo and before he joined Uriah Heep, he was in a band called Daddy in Aug 1969. Baker only spent a short time in this group. This band in early 1970 became Supertramp.
Terry Poole bass, lead vocals (born 19?? Chesterton Way? Leyfields) (ex May Blitz, to Graham Bond, Vinegar Joe);
Clem Cempson lead guitar, backing vocals, organ, harmonica (born David Clempson, 5.9.1949, in Bailey Avenue? Hockley? Tamworth) (to Colosseum, Oct 1969 - Aug 1970, Sep 1970 - Oct 1971, Humble Pie, Oct 1971 - Mar 1975, Strange Brew, Jul - Aug 1975 etc).
Not sure when John Hinch (born in 1948, at 98 Gala Lane, Lichfield, Staffordshire) joined the band? Hinch was in a band called Hiroshima (1972 - 1973), with singer Rob Halford (born Robert John Arthur Halford, 25.8.1951, in Sutton Coldfield, Warwickshire) who both went on to join Judas Priest in May 1973. Hinch left Priest in 1975.
As for Bakerloo they released an album 'Bakerloo' (Harvest SHVL 762, Dec 1968) and a 45 'Driving Backwards' / 'Once Upon A Time' (Harvest HAR 5004, 11 Jul 1968). As far as I know Baker played on the album as did one Jerry Salisbury on trumpet.

“Bakerloo” – a group which includes two Tamworth lads in it’s line-up – has a single and L.P. released within the next few weeks
Clem Clempson of Bailey Avenue, Hockley and Terry Poole of Chesterton Way, Leyfields, form part of the now famous “Bakerloo”.
The group is well-known in London and Birmingham and enquiries have been flooding record shops as to when the outfit will release a record.
The trio signed a recording contract on the newly formed E.M.I. “Harvest” label last week. They have a single called “Driving Bachwards” released on July 11 and an L.P. called “Gang Bang” five weeks later.
The trio have been together for 14 months and play a mixture of jazz, blues and classical music, giving their own interpretation.
Clem plays lead guitar and Terry is bass player and “Bakerloo” plan to play in Birmingham to promote the two releases.
Said manager, Jim Simpson, “The single is very commercial and very unlike their style, but it should get a lot of radio plays which will draw attention to the forthcoming L.P.”
